How to Log Patient Safety Reports
- Each hospital has its own safety reporting system
- If there is any question/confusion, please contact the QI Chief Resident and/or the Chief Resident at the hospital you are at. It is always good to let them know about any events as well. The safety posts are routinely reviewed by PHHS leadership and the chiefs. If you are interested in the outcomes of a patient safety post, please follow up with the chiefs.
- The patient safety post should not be used for any Title IX or sexual harassment reporting. This patient safety report process ideally helps identify systems issues that can be addressed to improve the safety of patients in the hospital. As such, anonymity when reporting is not guaranteed.
CUH
- CUH uses a system called RL6
- go to the UTSW clinical portal (http://clinicalportal.swmed.edu/)
- under Quick links on the left panel, click ‘Event reporting- use internet explorer’
- this will take you the RL6 system, log in with your UTSW ID/ password
- a colorful screen will show up:
