Introduction to ACR

elcome to the Ambulatory Care Rotation (ACR) for 2011-2012. The goal of the ACR rotation is for you to have exposure to a variety of medicine subspecialty clinics in order to learn about diseases and disease management not typically emphasized in the inpatient setting. We continually try to improve this aspect of your training. To that end, we have eliminated clinic experiences that were felt to be of low educational value, and we have added others that we feel offer excellent learning opportunities. We have also continued to add, as much as possible, more prolonged experiences in select subspecialties with the hope that you will use that time to focus on learning the material specific to that subspecialty that is traditionally tested on the ABIM certification exam. The ACR block is an excellent time for you to develop a study strategy that, if done correctly, will augment the knowledge you obtain on inpatient services in order to allow you to better care for your patients and successfully pass the boards.

Johns Hopkins Ambulatory Curriculum

The purpose of the ACR Friday workshop using the online Johns Hopkins Ambulatory curriculum is to improve the IM residency outpatient experience. The online curriculum consists of web based modules covering a wide range of common ambulatory topics. During the workshop, an assigned module will be discussed.

Clinic Attendance Policy

The primary goal of the Ambulatory Care Rotation is to provide interns and residents with exposure to a variety of outpatient settings where they can learn the fundamentals of primary care and the relevant outpatient issues associated with each of the medical specialties. Despite the traditional focus in internal medicine training on acutely ill patients in the inpatient setting, the majority of internal medicine and internal medicine subspecialty care is provided in the outpatient environment, so this opportunity should be taken seriously. It is expected that housestaff approach these opportunities with the same enthusiasm and accountability that they do an inpatient service. The clinics are aware of each housestaff’s assignment, and they create their schedules based on the assumption that the assigned housestaff will be present. Thus, your assignments are expected, not elective. Failure to attend assigned clinics without explicit permission from the chief residents and knowledge of the clinic staff will be viewed as unprofessional conduct in that it negatively impacts patient care and shows disrespect to the faculty and other trainees in the clinic.

Continuous Quality Improvement (CQI)

Awareness of the system in which one operates is an integral part of providing medical care today. Whereas in the recent past, an individual physician with a strong work-ethic and intellectual acumen could reasonably expect to obtain the knowledge and skills necessary to adequately care for their patient independently, this is not the world we live in today. Knowledge of disease processes, diagnostic technology, and therapeutic modalities has rapidly and vastly outstretched the ability of any one individual to keep pace. As a result, doctors and their patients are dependent on ever more complex medical systems to work for their benefit. It is incumbent upon the modern physician in training to gain the skills necessary to not only provide excellent care within complex medical systems, but to recognize when such systems are failing and understand how to participate in changing the systems to work as intended. Therefore, just as you will learn the skills and techniques needed to diagnose and treat illnesses in individuals that come to you for care during your residency, you will be expected to learn the skills and techniques necessary to diagnose and treat problems that exist in the system of health care delivery.

To this end, we have created a curriculum that is intended to teach you the concepts of quality improvement and patient safety. You will learn the fundamental principles of these fields in sessions such as ACR Friday Workshop and Patient Safety Conference. In the second half of the year, you will have the opportunity to use your knowledge of the system of healthcare delivery to develop a specific project intended to improve the care of patients in your continuity clinic.

2011-2012 ACR Schedule Template

Your assignment to each of the clinics on this schedule represents an agreement between that clinic and the residency program. The clinic staff go out of their way to assure adequate exam room space and assign patients specifically to you. You are expected to be in the clinics and to be on time. If you cannot make it to a clinic, you must find coverage. Failure to make the clinic and the chief residents aware of an anticipated absence or late arrival is considered unprofessional behavior and will be subject to formal reprimand.
